Membership & Pricing Information

James River Family Medicine operates under a Direct Primary Care (DPC) model designed to simplify healthcare and allow for more meaningful time between patients and their provider. Instead of billing insurance for routine primary care visits, patients enroll in a monthly membership that provides direct access to care and transparent pricing.

DPC Membership Plans

• Covers one child

• Covers one adult

• Covers up to two adults and dependent children living in the same household

One-Time Visit Options

Patients who prefer not to join the membership may schedule individual visits as needed.

In-Person Visit (Non-Member)

$125 per visit

Telehealth Visit (Non-Member)

$95 per visit

Telehealth visits may be appropriate for certain medical concerns and will be scheduled at the provider’s discretion.

Patients who anticipate needing ongoing care may find that a membership offers the most value.

Enrollment Fee

A one-time enrollment fee of $100 per patient will be charged upon joining the practice. This fee covers the administrative costs associated with establishing care, including patient onboarding, record setup, and account configuration.

For families enrolling together, a maximum family enrollment fee of $150 will apply.

Membership Commitment

Membership at James River Family Medicine includes a minimum six-month commitment when joining the practice.

This allows time to establish care, review medical history, develop a personalized care plan, and build a meaningful provider–patient relationship while ensuring continuity of care.

After the initial six-month period, memberships will automatically renew for additional six-month terms unless canceled prior to the renewal date.

Members who wish to cancel their membership may do so prior to the start of the next six-month term.

Membership Billing Policy

Membership fees are billed monthly and will automatically renew after the initial six-month commitment period.

Members are responsible for maintaining an active payment method on file. If a membership payment fails or is canceled before the initial commitment period is completed, the remaining balance of the commitment period may become due.

Memberships may be canceled after the initial commitment period with appropriate notice.

Membership includes:

• Unlimited visits as medically appropriate, whether in person or via telemedicine
• Online scheduling
• Longer, unrushed appointments
• Same-day or next-day appointments when available
• Telehealth visits when appropriate
• Direct messaging and communication with your provider through our secure messaging system
• Chronic disease management
• Medication management
• Preventive care visits
• Coordination of labs, imaging, and referrals when needed

Members also have access to discounted wholesale lab pricing through our partnered laboratories if they choose to utilize this option.

Services NOT Included in Membership

Membership does not include services such as:

• Laboratory testing (labs may be obtained through our discounted partner labs or through the facility of your choice where insurance may be billed)
• Imaging studies
• Medications
• Specialist visits
• Hospital care
  

Insurance Information

James River Family Medicine does not bill commercial insurance for routine primary care services. Patients are encouraged to maintain insurance coverage for services such as hospital care, specialists, imaging, and emergency services.

Medicare patients are welcome once credentialing is finalized, and visits for Medicare beneficiaries will be billed directly to Medicare according to federal guidelines.
